The Community Rehabilitation Team works with you to regain your independence, and improve your confidence and your ability to manage at home after an illness, operation or after you have had a fall or a traumatic event.
It can often be difficult to get your independence or confidence back, especially if you have been in hospital for a long time or have been trying to cope at home with difficulty.
We offer 24-hour support in 20 residential beds in a social service resource centre.
The service can also offer links to community services when planning your return home, and offer advice to your family, carer or friends.
You will be expected to fully participate in your recovery by practicing daily living activities and we encourage you to be as active as possible. Following your assessment, we’ll visit you at home to decide on your current and future needs.
The following people may be involved in your care during your rehabilitation stay:
- Social worker – to discuss any difficulties you may have at home, assess your needs and arrange the necessary support. Any charges for care at home will be discussed with you and your family.
- Geriatrician – to check your medical needs with the team.
- Dietitian – to work with the team to monitor your diet and provide advice on any changes needed to maintain balanced and healthy eating.
- Physiotherapist – to assess and work with you to help you regain strength, mobility, balance and confidence.
- Occupational Therapist – To assess and work with you to practice daily activities to help you return home.
How do I access the service?
All referrals to the service are through The Community response and Rehabilitation Team.
